CatSper test — when a standard spermogram is not enough

In some couples, everything seems “normal”. The spermogram is within reference limits, the ovarian reserve is good, the tubes are passable. And yet, pregnancy does not occur. In such cases, more subtle reasons are sought, hidden under the surface of standard research.

One of them may be dysfunction of the CatSper channels in sperm. And this is where the CatSper test comes to the rescue.

What is CatSper?

CatSper (Cation Channel of Sperm) is a specific ion channel in the sperm tail. It is responsible for the entry of calcium ions into the cell, which is essential for:
- hyperactivity of motility;
- navigation of the sperm to the egg;
- acrosomal reaction and fertilization.

If the CatSper ducts do not function properly, even a sperm with normal shape and motility may fail to fertilize an egg.

What is the CatSper test?

This is a specialized laboratory analysis that studies the functionality of CatSper channels in sperm. It is carried out by:
- seed sample;
- specific activation and evaluation of the calcium reaction;
- interpretation by an experienced specialist.

When is it recommended?

The CatSper test is particularly indicated in:
- unexplained male infertility;
- repeated unsuccessful fertilizations (IVF or ICSI);
- a high percentage of infertile embryos after ICSI;
- suspicion of a defect in the functional ability of the sperm.

What does the test show and what are the next steps?

If the test shows impaired CatSper function:
- the probability of natural fertilization or classical IVF is low;
- ICSI is recommended as a method that bypasses the need for active penetration;
- additional andrological or genetic evaluation may be discussed.

Why is this test important?

Sometimes the solution is not in another in vitro experience, but in a deeper diagnosis. The CatSper test can give an answer to the question “why it does not work” and direct treatment in the right direction.

Conclusion

The CatSper test is part of the new generation of functional assays of male fertility. It doesn't just measure number and movement — it measures the ability of a sperm to complete its task.

In our clinic, we apply the CatSper test in specific cases where we believe the answers lie beyond the standard spermogram. Because every “incomprehensible why” has its own hidden mechanism — and sometimes it starts with an ion channel.
